A review is to begin into the care of a six-year-old who fell from the eighth floor of a block of flats in Leeds.
Liam Shackleton died after falling from the building in the Lindsey Mount area on Tuesday evening. He was pronounced dead at Leeds General Infirmary.
Leeds Safeguarding Children Board said it would investigate the circumstances surrounding his death.
Two men aged 36 and 57 and two women aged 28 and 16 were arrested on suspicion of child neglect and bailed.
Jane Held, chairwoman of the Leeds Safeguarding Children Board, said: "We are saddened to hear about the tragic and untimely death of Liam Adam Shackleton.
"As is usual in such circumstances, the Leeds Safeguarding Children Board will now oversee a review into the circumstances of the death."
Ms Held confirmed social workers had not previously been involved with the family and windows in the block of flats "did incorporate safety measures" which met building regulations.
Jane Warden, 51, said she saw Liam on the ground after he fell.
"He was lying, face up, with his little arms and legs all twisted. There was no blood but his breathing was very shallow and his lips were blue.
'Very brave'"I knew then he was gone, no little kiddie could survive something like that.
"Around 10 people had gathered around and someone put a blanket over Liam until the paramedic arrived.
"He scooped Liam up into his arms and put him in the ambulance."
Neighbours said Liam moved into the flat in Lindsey Mount recently with his parents, Martin and Tanya Shackleton, two brothers and a sister.
One tribute left at the scene said: "Sweet dreams little one, love Jennifer, Paula and Mike."
Another card was written to a "very brave and well-behaved little boy".
